Diol epoxides formed by the sequential action of cytochrome P-450 and the microsomal epoxide hydrolase (mEH) in the endoplasmic reticulum (ER) represent an important class of ultimate carcinogenic metabolites of polycyclic aromatic hydrocarbons. The role of the membrane orientation of cytochrome P-450 and mEH relative to each other in this catalytic cascade is not known. Cytochrome P-450 is known to have a type I topology. According to the algorithm of Hartman, Rapoport and Lodish [(1989) Proc. Natl. Acad. Sci. Es wird uber einen Pall einer suicidalen Vergiftung durch subcutane Injektion der insecticiden Kombination von Parathion (E 605®) mit Demeton-O-methyl-sulfoxid (Metasystox R®) berichtet. Nach einer akuten Phase, die hauptsachlich durch muscarinahnliche Symptome gekennzeichnet war, schritt die Vergiftung bis zu einer peripheren Atemlahmung nach 6 1/2 Tagen fort. Der klinische Verlauf wird unter Berucksichtigung der Toxicitat der Giftstoffe, ihres Resorptionsweges, ihrer Dosis und der durchgefuhrten Behandlung besprochen. Es wird angedeutet, das die kombinierte Toxicitat der Giftstoffe fur den Menschen hoher sein konnte, als nach fruheren Hinweisen zu erwarten war.

Es wird uber drei todlich verlaufene Zwischenfalle mit einer uberlebenszeit von 21–46 Std nach einer intravenosen Injektion von angeblich 1/8 mg Strophanthin, 2 ml Vitamin B-Komplex und einem Ro\kastanienextrakt (2,5 ml) berichtet. Die Ermittlungsergebnisse, das klinische Bild, die Sektionsbefunde und die feingeweblichen Untersuchungen lassen jedoch eine Invertseifenlosungintoxikation wahrscheinlicher erscheinen. Die bisher in der Literatur niedergelegten pharmakologisch-toxikologischen und klinischen Erfahrungen mit Glutethimid („Doriden-CIBA“) werden mitgeteilt und diskutiert. Die Gesamterfahrungen rechtfertigen die Verwendung dieses neuen Arneimittelkorpers als Schlafmittel. Eine Ausnahmestellung gegenuber anderen leicht abbaubaren Schlafmitteln der Barbitursaurereihe wird nicht ersichtlich. Es wird deshalb die Aufnahme von Glutethimid in die Liste der rezeptpflichtigen pharmazeutischen Spezialitaten fur notwendig erachtet.
